About the Role

Taykwa Tagamou Nation is seeking a highly motivated, confident, organized professional individual to join the Health Department. Under the direction of the Director of Health, the Health Navigator/NHIB Service Worker will coordinate and deliver outreach strategies, identify community members, and work internally with fellow staff. They will also be responsible for ensuring assistance is provided in health navigation and support with the NIHB services, will work to improve and enhance the quality of life for all ages in Taykwa Tagamou Nation.


Role & Responsibilities

•       Navigate by assisting client with medical appointments: to improve client care outcomes, improve access, transitions and guide with care planning, care pathways, health literacy support digital access to care information and assist clients in decision making to improve health outcomes.

•       Engage clients through supportive, empathetic, open-minded, and non-judgmental communication and support.

•       Effectively assessing clients’ needs and co-developing solutions with the client through a trauma informed and culturally safe practice.

•       Balancing effective and outcomes orientated solutions in a dynamic fast-paced environment and with teamwork and collaboration to ensure positive outcomes.

•       Coordinate and assist clients with e-health services and consult/collaborate with nursing health team.

•       Ensure financial policies and procedures are followed

•       Engagement and relationship building; nurture relationships across community-based programs and services; services providers; and First Nations, federal, provincial and territorial programs and services, and identify/develop possible models of service delivery that will improve timely access to services for First Nations children living in or outside their communities

•       Other duties as assigned.

 

Qualifications

•       Minimum of Ontario Secondary School Diploma or equivalent.

•       Combination of education and/or experience in health, social work or community work or related fields.

•       Minimum of two years’ working experience in sectors: health, social work or community work.

•       Be solutions focused on providing services and results oriented in the role of the navigator.

•       Knowledge of Aboriginal culture and experience working with aboriginal people

•       Must possess a valid G driver’s license

•       Advanced knowledge and experience with productivity applications such as Microsoft Office, Excel, Outlook, MS Teams and Power Point.

•       Knowledge of NHIB and their processes.

•       Must provide a recent CPIC which includes the Vulnerable Sector Screening or proof of applying for one

•       Excellent organizational and time management skills

•       Understanding and knowledge of the Taykwa Tagamou Nation and the services they provide.
